NEEDS HELP WITH ACTIVITIES OF DAILY LIVING

I have a Unit owner who is ill and has his son staying with him for the next few months, What form do I need to have filled out by our association and stamped with our seal, to have a pass issued for the year or so? Please advise

    Hi Nina,
    Have the Board get the application(s) from UCO. Have him fill it out and be investigated. then, if the results are ok, issue him a Certificate of Approval and the ID office will issue him the Occupant ID card.
